Under 18 Consent Form
& Parent Agreement

This agreement outlines the expectations and responsibilities regarding your child’s participation in Solution Focused Hypnotherapy sessions. By signing this agreement, you acknowledge and accept the terms outlined below:

1. Commitment, Standards of Behaviour and Code of Conduct

​

1.1. Throughout your child's therapy sessions, NC Hypnotherapy will commit to acting in their best interests, to treat them with respect, including respecting their privacy and confidentiality at all times. 

 

1.2. Hypnotherapy relies on the commitment and motivation from both the therapist and your child to gain the best results. Part of the therapy occurs in the therapy room, but a vital part of the process involves reflection and carrying out tasks outside of the session, including listening to the hypnotherapy audio at appropriate times and working on the 3Ps.  It is with commitment and partnership that your child will see the best results.

1.3. Unless prior agreement is made, NC Hypnotherapy will not make any contact outside of sessions (unless for rescheduling or cancellation purposes).

 

1.4. Clients should not attend sessions under the influence of alcohol or recreational drugs. NC Hypnotherapy reserves the right to terminate the session and refuse any further sessions, if the client shows inappropriate behaviour.

1.5. To maintain a focused and distraction-free environment, all mobile phones must be switched off or set to silent during sessions.

1.6. It is advised that you consult your child's doctor before having complementary therapy, including Hypnotherapy.

​

1.7. NC Hypnotherapy is registered with the Association for Solution Focused Hypnotherapy (AfSFH), National Council for Hypnotherapy (NCH) and Complementary and Natural Healthcare Council (CNHC) and abides by their code of ethics. 1.8. NC Hypnotherapy is based in the UK, under English and Welsh law, and governed by UK jurisdiction.

​

2. Hypnotherapy Sessions and Fees

​

2.1. Each Hypnotherapy session will last between 50 and 60 minutes. There is no waiting room and parent/carers are welcome to leave and promptly return or wait outside. Details of fees can be found on the page headed 'Sessions & Prices' at the top of this page.

 

2.2. If you are running late or anticipate running late for your session, please leave a message (calls are not answered when working with a client) or send a text to 07841 018533. NC Hypnotherapy will not be able to see you for more than your allocated time slot due to scheduling and commitments to other clients. 

 

2.3. A committed and motivated client is likely to achieve their realistic goals. NC Hypnotherapy does not guarantee 'cures' as it would not be ethical to make this claim and it is important to note that results may vary from person to person.

 

2.4. A guide to therapy duration and the number of sessions required will be provided at the Initial Consultation however this will be regularly reviewed during your therapy and any changes to this will be discussed with you. The number of sessions are estimates based on experience with previous clients, however as all clients and conditions are unique, these estimates are subject to change.

 

2.5. Session fees must be paid before the session otherwise NC Hypnotherapy has the right to cancel the session.  Payment can be made by credit/debit card, bank transfer or cash. Bank details are available on request.

3. Online Sessions

​

3.1. Sessions will take place via a secure online platform (usually Zoom) and the meeting link will be provided in advance.

 

3.2. Sessions should take place in a quiet, private space where your child will not be disturbed. Please ensure that there is a stable internet connection, a working microphone and camera.

 

3.3 Online sessions are not to be recorded. NC Hypnotherapy will not be recording any of our sessions UNLESS there is prior consent and this has been mutually discussed and agreed and a consent form has been signed by you. 

 

3.2 If the connection is lost during hypnosis, your child will remain in control and they will be able to gently bring themselves back to full awareness. NC Hypnotherapy will do their best to contact you using the contact details provided (email or telephone).

4. Cancellation Policy

​

4.1. NC Hypnotherapy's cancellation and rescheduling policy requires that at least 24 hours notice is given. Less than 24 hours notice will result in a full charge for the session or a deduction from a block of sessions. An invoice will be issued to you via email. All invoices must be paid within 7 days.

 

4.2. NC Hypnotherapy reserves the right to cancel an appointment without notice if the room or environment is unsuitable for the treatment to be provided.

4.3. If an emergency requires NC Hypnotherapy to cancel your appointment, as much notice as possible will be given to you. If this notice is less than 24 hours and you have paid in advance a full refund will be issued to you.

​

4.4. Should NC Hypnotherapy need to cancel any appointment they will commit to scheduling a new appointment at the earliest convenience of the therapist and the client.

 

4.5. If NC Hypnotherapy needs to contact you they will do this via your preferred method as indicated on your initial consultation form. If a message is to be left no mention of hypnotherapy will be made.

5. Contact Outside of Sessions

​​

5.1. Contact in between sessions is best kept to booking or re-arranging appointments, and brief queries or clarifications, unless otherwise agreed in the initial consultation. 

​

5.2. In instances where you need to contact NC Hypnotherapy between sessions an email can be sent to nicole@nchypnotherapy.co.uk  Messages will be responded to during office hours (9am to 5pm, Monday to Friday), generally within 1 business day.

 

5.3. NC Hypnotherapy is not a crisis or emergency service and does not provide out-of-hours or emergency cover. In the event of a crisis or emergency, please contact your GP (out-of-hours dial: 111), attend your local A&E department, or dial 999. You may also find the Samaritans (116 123, 24 hour support) or Childline (0800 1111) helpful.

​6. Privacy and Confidentiality 

6.1. Your privacy is of the utmost importance to NC Hypnotherapy. Any information that is collected from you or your child, including contact details is held in the strictest of confidence and will never be shared with external parties without your express permission with the following exceptions outlined in 6.3 and 6.4.

 

6.2  NC Hypnotherapy will not discuss the content of the sessions with parents, unless the young person requests or gives consent for this and with the exceptions of 6.3 and 6.4.


6.3.  NC Hypnotherapy has a professional obligation to report to relevant authorities any concerns where they believe the client may intend to harm themselves, the therapist, or others.

6.4. As part of NC Hypnotherapy's professional practice, they will attend regular supervision sessions and cases may be discussed with other qualified hypnotherapists. Your child's identify will not be disclosed and no personal details will be shared.

6.5 In compliance with GDPR guidelines and for insurance purposes, all notes from our sessions will be held for eight years from the date of their last visit or, if the client is a child, until their 25th birthday, or 26th birthday if the client was 17 when the sessions ended.

Parent Agreement

​

​​The purpose of this agreement is to outline the key principles that guide my therapeutic practice, ensuring you have accurate and informed expectations when considering Solution Focused Hypnotherapy for your child or adolescent.

​

  • Confidentiality and safeguarding

 

An essential aspect of effective therapy is maintaining confidentiality regarding the information shared. This allows the young person to express their feelings openly without fear of judgment or consequences. Confidentiality also means that details of the therapy sessions will not be shared with parents unless the young person specifically requests or consents to it.

 

I acknowledge that this is not always an easy situation for parents and it is quite natural for you to feel anxious about what may be shared in the sessions, but ensuring the confidentiality of our work is crucial for establishing trust. This enables the young person to feel confident to speak openly and freely about what is bothering them.

 

Although the contents of the sessions will be confidential, I retain the right to breach confidentiality in some specific circumstances outlined in the terms of conditions clauses above (6.3 and 6.4)

  • Hypnotherapy Sessions

 

The number of sessions needed for client will vary depending on the complexity of the challenges that they are experiencing. Every sixth session, there will be a review of our work and we can discuss whether further sessions are required, including their frequency.

 

If your child arrives for the session under the influence of drugs or alcohol, the agreed session will not take place and you will still be liable to pay the full session fee.

 

I understand that your child’s life circumstances may suddenly change, and your child may at any point wish to discontinue therapy. I will always respect their decision but ask that I am given one session's notice before finishing so that we have the chance to discuss this decision and end the work accordingly.

  • Contact between sessions

To ensure professional boundaries and the smooth running of sessions, I kindly ask that all communication outside of scheduled sessions is limited to booking or rescheduling appointments. I am unable to respond to messages or calls regarding session content or other matters outside of our agreed appointment times. 

​

I am available Monday to Friday from 9am to 5pm and will respond to you within 24 hours or the next working day.  Please understand that my therapeutic relationship is with your child and therefore it is helpful if you let your child know what information you have shared with me.

  • Supporting your child

​

Your child has certain responsibilities within the course of Hypnotherapy. The success of the treatment, in part, is determined by their desire to achieve the changes and the commitment to the format of the sessions.

​

To create a safe and open space for your child, I encourage parents to avoid asking them for specific details about what was discussed during sessions. Instead, I recommend focusing on and positively reinforcing any changes or progress you observe. This helps build their confidence and allows the therapeutic process to be as effective as possible.

​

By encouraging and supporting them to listen to the relaxation audio each night and participating in activities of positivity between sessions, you will optimise their chances for maximum benefit from each session and the therapy as a whole.
